Welcome to your Summerlin South Little League's 2023 All-Star Commitment Application.  When filling out this Application, please answer all of the questions.  Also, this application requires you to upload PDF copies of the player's birth certificate and certain proofs of residence.   Please follow the instructions closely as there are very strict requirements on the dates and types of documents used for registration.

If the player attends a school located in the league's boundaries (listed on the letter that brought you to this website), please upload a School Enrollment Form completed by the player's school.  The school enrollment form is available here.  This will be the primary form of residency proof we use this season.

If a player does not attend a school in the league's boundaries, the player must reside in the league's boundaries and we will require three specific forms of address proof be uploaded with this application.   These documents must be dated between February 1, 2022 and February 1, 2023 (current year) that show the parent’s name and address from EACH of the following categories:

Group I:

1.         Driver’s License (preferred document)
2.         School records
3.         Vehicle registration or lease
4.         Employment records (paystub, W-2, etc.)
5.         Insurance documents

Group II:

1.         Welfare/child care records
2.         Federal records (tax, social security, etc.)
3.         State records (including DMV registration) (preferred record)
4.         Local (municipal) records
5.         Support payment records
6.         Homeowner or tenant records (deed, rental agreement, etc.)
7.         Military records

Group III:

1.         Voter’s registration
2.         Utility bills (gas, electric, water/sewer, phone, waste disposal, etc.) (preferred record)
3.         Financial (loan, credit, investments, etc.) records
4.         Medical records
5.         Internet, cable or satellite records

NOTE: Three documents from one category  constitute only ONE document.
